1. What We Collect on Website Forms
Our website enquiry forms collect only basic coordination details: visitor name, phone number, locality/neighbourhood in Shivamogga, service of interest, and message. We do not collect medical history, diagnosis files, or Aadhaar numbers through online contact forms.
2. Storage & Encryption
Inquiries are stored securely in a dedicated local database table (uak_ts_enquiries) protected by WordPress server-side access controls.
3. Retention Period
Inquiry records are retained for up to 12 months for callback and service audit verification, after which completed inquiries are archived or purged.
